QUALIFICATION

Any natural person can be a member of the PHCCI-MPC Tacloban provided he/she possess the following:
           Must be 18 years of age
           Must be a Filipino Citizen
           If living abroad, must be a natural-born Filipino Citizen with permanent address in Eastern Visayas
           Must be able to validate enter into a contract

Online Application

PHCCI-MPC Tacloban accepts online applications for membership which a prospective applicant can do so through its membership Online Application facility.

Fill-out the online form and upload the scanned/clear picture of the following requirments:

                                •   Barangay Certificate of Residency (certificate must be from the Brgy. of your residence)
                                •   Proof of Billing (optional)
                                •   Selfie picture holding your Valid ID
                                •   Specimen Signature (front and back)
                                •   House Sketch (Note: Draw in a clear sheet bond paper and kindly make it clear and easy to locate)
                                •   Deposit Payment Slip
                                •   TIN ID or TIN No.

Wait for a confirmation of your application submission
Once application is confirmed, you will be informed to attend the online Pre-Membership Education Seminar every Saturday at 9:00AM via Zoom application.

Onsite Application

A membership applicant may opt to open account onsite by visitng the nearest PHCCI office.

Submit the following documentary requirements:

                                •   Barangay Certificate of Residency or Latest and Original proof of billing under the name of applicant
                                •   TIN No. or Photocopy of TIN ID
                                •   Photocopy of one (1) Government issued ID or School ID for student applicant
                                •   2 pcs. 2x2 ID Pictures
                                •   Deposit and Fees

Attend the Pre-Membership Education Seminar every Monday, Tuesday and Saturday at 9:00AM.

Rights & Duties

Rights of Membership

     •  Equal voting rights with other members regardless of
         the amount of share capital contribution.

     •  To avail of all services and facilities of the COOP such
         as loans, tax-free interest income on deposits,
         whether savings or time.

     •  To be informed of the financial status of the
         cooperative

     •  To attend and participate in all general and special
         meetings for the membership as loans, tax-free
         interest income on deposits, whether savings or time.

     •  To have access to information on all policies,
         guidelines rules and procedures affecting
         membership rights and interests.

Duties & Responsibilities of Membership

     •  To pay promptly the annual membership dues

     •  To settle promptly all outstanding obligations with the
         COOP

     •  To maintain a current record of all personal data in
         the PHCCI database

     •  To be regularly informed of new policies, procedures,
         news or events concerning COOP matters by regularly
         accessing the COOP’s website and online social media
         pages.
